This page reports what four public programs have published about themselves, read on the date in the hero. It is not advice, it is not an eligibility determination, and it cannot tell you whether a particular employer or job qualifies. Only the program can do that.

None of it can be guaranteed current. Program rules change, award amounts change, application cycles open and close, and shortage-area designations are added and withdrawn continuously — of the 5,017 California mental health designations on file right now, 4,197 are already withdrawn or proposed for withdrawal. A figure that was right the day it was read can be wrong the day you read it.

Verify directly with the government before you make an employment decision, sign anything, or change a repayment plan. Every program below links to its own official checker: the federal PSLF employer search, HRSA’s shortage-area finder, and HCAI’s eligibility quiz. Use them. Getting this wrong can cost years of qualifying payments, and no page can carry that risk for you.

The comparison

They are not four versions of the same test.

Each program asks a different question, which is why the same job can qualify under one and be ruled out by another. The column people never see is the third one.

A fifth, the Licensed Mental Health Services Provider Education Program, pays up to $15,000 for a 24-month commitment and can be awarded 3 times in a career. Its program page does not state whether pre-licensed registrants qualify, so this page does not say either way.

PSLF has no cap at all — it forgives the remaining balance — and MBH-SLRP is the largest fixed award of the four. The programs that rule you out are the smaller ones.

2The two that rule you out say so in terms.

NHSC requires you to be “fully trained and licensed to practice” in the discipline and state. California’s SLRP requires you to “possess a valid and unrestricted license”. Neither has an associate tier, and no amount of supervised experience substitutes.

3Which means the question to ask an employer changes with your status.

Pre-licensed, you are asking whether they are a government body or a 501(c)(3), and whether they are a Medi-Cal safety-net setting. Licensed, the site’s shortage-area designation opens up as well — and for CA SLRP, whether the site will put up matching funds, which is the condition that quietly disqualifies most of them.

PSLF

The federal one does not care what your license says.

The regulation defines a qualifying employer by what the organization is, not by what you do inside it. There is no discipline list, no license requirement and no job title in the definition.

Two consequences almost nobody draws

An associate at a county behavioral health agency is very likely accruing qualifying payments right now. The county is a local government entity, which is the whole test. And on income-driven repayment at associate pay the monthly payment is small — so the balance forgiven at the end is large relative to what was actually paid. Nobody tells you this at induction.

An associate at a for-profit group practice serving Medi-Cal clients qualifies for none of the four. Serving the right population does not help: PSLF asks about the employer, and a for-profit practice is a business entity. This is the opposite of what most people assume, and it is the single most expensive assumption on this page.

If you think this may already apply to you, the thing to do is not to read more about it — it is to run your employer through the federal employer search and file the employer certification. Certifying early is how you find out you were wrong while it is still cheap.

The California programs

One names associates. The other requires a license and a willing employer.

Medi-Cal Behavioral Health Student Loan Repayment — the one that reaches you

Awards are stated as maximums. The service obligation must be completed in a Medi-Cal safety-net setting: a federally qualified health center, community mental health center, rural health clinic, or a setting where at least 40% of the population is on Medicaid or uninsured — 30% for rural hospitals. The next application window opens 1 May 2027.

California SLRP — licensed only, and the match is the catch

$50,000 for a two-year full-time obligation, $25,000 half-time, then extensions of $20,000 a year and later $10,000. Eligible disciplines include LMFT, LCSW, LPCC and health service psychologist. The applicant must “possess a valid and unrestricted license”, so an associate cannot apply.

The site condition is the one that decides most cases. It must sit in a federally designated shortage area, be a public or private not-for-profit outpatient facility open to the general public, offer a sliding fee schedule, and match the award dollar for dollar. That last requirement is why a designated shortage area near you is not the same thing as a participating employer near you, and why this is a question to ask the employer rather than to infer from a map.

HRSA publishes every mental health shortage-area designation it has ever recorded. Most of California’s are no longer in force, and reading the file without filtering on status overstates the state by about six times.

A facility-type designation — health center, rural health clinic, tribal health — attaches to a named site. A population or geographic designation attaches to an area, and a job inside that area is not automatically at an approved site.

The shortage score runs from 3 to 24 across California’s live designations, averaging 16.6. It matters because NHSC funds applications from the highest scores down, so two approved sites are not equally likely to get you an award.

Every county but one has a designated shortage area.

The only California county with no designated mental health shortage area is Alpine — also the state’s least populous. Health center sites are counted separately because they are named facilities rather than areas, and they are the clearest overlap between the federal and state programs.
