Use it this way

Use every week you claim experience and before asking a supervisor to sign a log.

Do not mistake it for this

This is not a BBS weekly log, supervision agreement, or determination that hours qualify.

Working resource

Make the invisible facts visible.

Review point 01

Separate each work setting

Record hours and supervision by setting rather than combining employers into one apparently compliant week.

Review point 02

Classify the experience

Use the current Board categories for your registration type; do not create a private category that cannot map to the application.

Review point 03

Record direct-clinical work

Keep the client-contact subtotal visible and supported by the contemporaneous practice record.

Review point 04

Match supervision to the week

Record the date, format, duration, and supervisor for the contact supporting that week.

Review point 05

Flag unusual weeks

Mark absences, multiple settings, high-hour weeks, or changes for review before signature.

Review point 06

Reconcile and preserve

Compare your copy with the signed official log and preserve the controlling form, not only this worksheet.

Private working copy

Carry the review into your own next action.

Private by design

Your entries remain only in this open page. They are not saved, uploaded, transmitted, or placed in the URL. Use a non-identifying reference—not a client name.

Closing or reloading this page clears these fields.

Controlling material

Read the authority, not merely the worksheet.

Educational information only; not legal, clinical, tax, billing, or regulatory advice. A resource cannot resolve facts it was not given or authority that changed after the review date.

You should not have to work this part out on your own.

Once a month: free tools and apps worth having, better ways to run the admin side of a practice, what other California therapists are actually doing, and anything new here that might save you an afternoon.

About monthly. One click to leave. Never sold, never shared. The consent box is separate and unticked because California requires it — and because it should be.
