The Board’s paperwork for the MFT path: sixteen forms, and which ones you keep.

From the associate application to the license issuance, each Board form with its number and revision date, who completes it, whether it is kept or submitted, and the regulation that makes it so. The weekly log is kept and never sent unless asked; the Supervision Agreement is signed within 60 days, kept, and sent with the license application; the oversight letter is signed before the first hour. The fees are the ones printed on the forms.

Form by form

Sixteen documents in four stages, from the Board’s LMFT applicants page.

Each row links the Board’s PDF as posted on 5 September 2026. The number and revision date are the ones printed on the form; if the Board’s page shows a later revision, the Board’s copy governs. The LCSW and LPCC paths use their own forms on the same pattern and are not listed here.

The fee calculator carries the same figures.

The pattern

One regulation decides which forms you keep and which you send.

§1833(d)The weekly log is kept, signed every week, and produced only on demand.

“Supervisees shall maintain a weekly log of all hours of experience gained toward licensure. The log of hours shall be signed by the supervisor on a weekly basis. The supervisee shall retain the signed logs until such time as the supervisee is licensed by the board. The board shall have the right to require an applicant for licensure to submit all or such portions of the logs as it deems necessary to verify hours of experience.” The form itself says: do not submit unless specifically requested.

§1833(c)The Supervision Agreement is signed within 60 days, kept, and sent with the application.

“Within 60 days of the commencement of any supervision, the supervisor and supervisee shall sign a supervision agreement … under penalty of perjury. The original signed supervisory agreement shall be retained by the supervisee and submitted to the board with the supervisee’s application for licensure.” Relationships that began before 1 January 2022 use the responsibility statement and a supervisory plan instead ((c)(2)).

§1833(a)The oversight agreement is signed before the first hour.

Where the supervisor is not the employer’s employee, or volunteers: “The written oversight agreement shall be executed and signed prior to commencement of supervision”, the supervisor provides it to the supervisee at the end, and “the supervisee shall submit the agreement to the board upon application for licensure.” The Board’s sample is a letter on the employer’s letterhead.

§1833(e)The hours are documented once, at the end, by the supervisor.

“Completed hours of experience shall be documented at the completion or termination of supervision”, with the supervisor’s license and telephone, the employer, whether the setting complied with §4980.43.3 and §4980.43.4, employment or volunteer status, the hours by category, the supervision provided, the dates, and “the supervisor’s signature under penalty of perjury.” That is the Experience Verification, 37A-301.

§1833.1(b)A supervisor who stops signing owes a week’s written notice.

“A supervisor shall give at least one (1) week’s prior written notice to a supervisee of the supervisor’s intent not to sign for any further hours of experience for such person.” The log and the agreement are what the notice is about.

Every form was downloaded from bbs.ca.gov on 5 September 2026 and its own instructions read; numbers, revision dates and fees are as printed. Forms are revised without notice, and the Board’s page is the copy that counts. This page covers the MFT forms; the LCSW and LPCC sets follow the same regulation with their own numbers.
