Can a California therapist apply?
What you need before applying
- An active California professional license appropriate to the services you will bill.
- An individual Type 1 NPI. A group or professional corporation may also need a Type 2 NPI.
- A complete CAQH profile, with Anthem authorized to view it.
- Current professional-liability coverage, W-9, service locations, work history, specialties and telehealth information.
- A practice address and counties you can actually serve. Network need can differ by specialty and geography.
The Anthem application sequence
- Prepare NPI and tax information. Make the legal name, billing name, TIN and NPI relationships consistent.
- Complete and attest CAQH. Upload current documents and authorize Anthem before submitting the payer request.
- Select the program, California and your provider type on Anthem’s enrollment page. Follow the displayed route and save any confirmation and submission date.
- Answer completeness requests immediately. The statutory clock applies to a completed application, so an unanswered request can stop it.
- Review the contract and fee schedule. Credentialing approval does not by itself establish a participation date.
- Wait for a written effective date. Do not represent yourself as in-network or bill as participating before it arrives.
How long should it take?
Source scope rechecked 8 September 2026. Anthem’s California Medi-Cal provider site gives a typical 45-day estimate after completed CAQH reaches its credentialing department. This is a source-specific estimate, not a verified timeline for every commercial product. Ask the selected program for its current process. California separately requires plans to acknowledge a mental-health credentialing application within seven business days and assess and verify qualifications within 60 days after receiving a completed application. The law expressly excludes contracting completion from that 60-day clock.
Reimbursement and the contract
Anthem does not publish a California therapist fee schedule for prospective providers. Rates are contract-specific and may vary by network, product, service location, license, code and negotiating arrangement. Ask for the allowed amount for the codes you actually use—commonly 90791, 90834, 90837, 90846 and 90847—before signing.
Separate the direct Anthem contract from participation through a third-party platform. A platform may give access to Anthem-funded members, but the platform is normally the contracting and payment intermediary. Compare payment, claim handling, credential portability, termination terms and whether the relationship follows you if you leave.
Common administrative failures
- Anthem cannot see the CAQH profile because authorization was never granted.
- The CAQH attestation or liability certificate expires during review.
- The W-9, NPI registry and application use different legal or practice names.
- A group application is submitted when the therapist still needs individual credentialing.
- The clinician assumes credentialing completion equals an active contract.
- The fee schedule is accepted without checking the highest-volume CPT codes.
Run a realistic Anthem scenario
Do not compare only the session fee. Model 12 weekly Anthem sessions, your offered allowed amount, expected no-shows, annual CAQH and claims administration, billing fees and unpaid follow-up time. Compare the resulting hourly net with the number of private-pay sessions you can realistically fill.

- California Health & Safety Code §1374.197—mental-health credentialing acknowledgment and review periods.
- CMS NPPES—NPI application and maintenance.
Review scope: Enrollment routing and the cited timeline’s program context were checked on 8 September 2026. Other source dates remain unchanged. Rates: Commercial contracted rates are not treated as public facts. Verify the network, fee schedule and effective date in your own written contract.
