Joining Blue Shield of California as a therapist

Blue Shield began directly managing behavioral-health credentialing in 2026. The packet, rates and signed agreement come before credentialing—and that order changes how you should approach it.

The short answer

Blue Shield accepts behavioral-health applications directly. It says a completed packet may take 45–60 days to process. The packet must be current, and its published workflow places the signed contract and rates before credentialing begins.

What changed in 2026

Magellan states that it stopped managing behavioral-health services for Blue Shield of California members on January 1, 2026. Older instructions that send an independent therapist to Magellan can therefore be stale. Start with Blue Shield’s current prospective behavioral-health provider page.

Application sequence

  1. Download the current behavioral-health packet. Blue Shield says the application must be dated no more than 30 days before submission.
  2. Prepare NPI, W-9, license and liability documents. Match the individual and entity structure.
  3. Complete the contract and proposed rates. Blue Shield’s workflow says the completed application, signed contract and rates are submitted before credentialing starts.
  4. Submit by the current published channel. Retain the entire sent packet and delivery confirmation.
  5. Answer credentialing requests. Track the date Blue Shield confirms completeness.
  6. Wait for the written effective date. Blue Shield warns that services before it process as out of network.

How to approach the fee schedule

Because rates appear in the pre-credentialing packet, decide your minimum acceptable amounts before submission. Ask for or verify the schedule for 90791, 90834, 90837, 90846 and 90847; identify product-specific differences; and confirm telehealth billing rules and place-of-service requirements.

Negotiation moment: the packet is the point to ask for a different rate—not months after loading the panel into your calendar. Commercial rates are not public, and the only controlling figure is the one attached to your agreement.

Common failures

Blue Shield scenario

Model the offered fee at 10 sessions per week. Add 90 minutes of monthly eligibility and claim work, your billing cost, no-show assumptions and the possibility that the panel fills gradually over three months. Compare the annual net with ten private-pay slots at your realistic—not ideal—fill rate.
