Can California LMFTs enroll?

Yes. Medicare began covering services furnished by marriage and family therapists and mental health counselors on January 1, 2024. Enrollment is through PECOS or the paper CMS-855I; California’s Medicare Administrative Contractor is Noridian.

Who qualifies

CMS requires the applicable master’s or doctoral degree, state licensure and at least two years or 3,000 hours of post-master’s clinical supervised experience. “Mental health counselor” is a Medicare category that can include qualifying California LPCCs; do not rely on the title alone—use CMS’s education, license and experience definition.

PECOS enrollment sequence

  1. Create or confirm your I&A account. This controls access to Medicare enrollment systems.
  2. Confirm your individual Type 1 NPI. Update NPPES before PECOS if legal name or address information is stale.
  3. Choose participating, nonparticipating or opt-out status. Understand assignment and private-contract consequences before filing.
  4. Submit the CMS-855I through PECOS. Reassignment information is now handled within the enrollment workflow when applicable.
  5. Respond to Noridian requests. CMS instructs applicants to answer contractor requests within 30 days.
  6. Wait for approval and effective-date confirmation. Then set up claims, remittance and beneficiary cost-sharing workflows.

What Medicare pays

For covered MFT and MHC services, Medicare pays 80% of the lesser of the actual charge or 75% of the amount determined for a clinical psychologist under the physician fee schedule. The practical allowed amount varies by CPT code and California locality, so use the current CMS Physician Fee Schedule—not a national anecdotal rate.

For the seven psychotherapy codes in each of California’s 29 localities, the Medicare payment calculator runs the fee-schedule formula on CMS’s 2026 files, applies the 75% rule, and splits the result into Medicare’s 80% and the patient’s 20%.

Medicare’s public schedule makes it a useful benchmark when evaluating commercial offers, but it is not interchangeable with a commercial contract rate.

Telehealth and opt-out decisions

CMS’s February 2026 telehealth guidance permits behavioral-health telehealth in the home and audio-only services when requirements are met. CMS states that the behavioral-health in-person requirement begins January 1, 2028 under the current extension. Confirm the current rule at the date of service because Congress can change these deadlines.

Opting out is not simply declining assignment. It creates a private-contract framework and generally renews automatically in two-year periods. Review CMS and Noridian instructions before choosing it.

Medicare caseload scenario

Use the current locality-specific allowed amount and model six weekly Medicare sessions. Include beneficiary cost-sharing collection, claim administration, secondary coverage and the public rate update each January. Compare participating and opt-out structures only after understanding their legal consequences.
